Taped in one of front pages of log book is a letter from the Division of Mental Diseases to Dr. Ivan Sletten dated Dec. 17,1968. The subject of this letter is suicides committed since July of 1967.
Death information:
Date of death, first admission #, re-admission #, name, county, co, pvt, physician, sex, race, religion, age at admission, date of admission, age at death, total time in hospital (years, months, days), adm. # admitted for month/year, mental code #, cause of death, death ICD code, autopsy, coroners case, anatomical, place of burial.
In Oct. 1967 a column was added after cause of death for date of autopsy report & ward was added before physician,
In Sept. 1968 the following columns were omitted:
