Date expired, chart #, sex, name, physician signing death certificate, date signed, funeral home, person claiming body, fathers name, mothers name, date admitted, time in hospital (yrs, months, days), time on books (yrs, months, days), home county, race, age, coroners case, anatomical, APA/DSM code, death codes, cause of death.
Each month has a separate medical record department sheet which has names & information about the deaths for that month.
In April 1981 (page 48) physician signing death certificate, date signed & mother & fathers names were removed from the headings & status, clinician, place of burial & next of kin were added.Collection
