Male & Female Patient Log Book for deaths from May 1,1970 thru June 30,1979.
Death information:
Date of death, case/chart number, name, county, unit expired in, home unit, physician, sex, date admitted, age at death, total time in hospital (years, months, days), APA code, ICDA code, # of admission for month & # for year (in hospital or on leave), cause of death, autopsy, autopsy report, anatomical, coroners.
In July 1978 the headings started including funeral home, home county, date of birth, age, race, mothers name, fathers name, death codes & cause of death written out. Physician was changed to physician signing death certificate & added date death certificate was signed. In April 1979 a heading was added for person claiming the body.
At the end of each month they have the totals for the following:
Deaths in hospital
Deaths on leave
Autopsies for the month & year
Total hospital days
Average length of stay
These totals were written at the bottom of the page until Sept. 1974 when they stopped. They resumed in March 1978 but were written on a separate piece of paper from the medical record department. This sheet of paper compiled the information concerning the deaths for each month. These papers are loose in the log books & placed in the page for each month.Collection
