Male & Female Patient Log Book for discharges from July 1, 1968 thru June 30, 1972.
Discharge information:
Date, case # (for first admission or re-admission), county, support (county or private), physician, sex, race, religion, age at admission, date admitted, age at discharge, time in hospital ( years, months, days), admission for the month & year, discharge form hospital or leave, diagnosis code.
The following columns were deleted in Sept. 1968:

Race
Religion
Date admitted
Diagnosis code. This was added back on in July 1971
A column for unit was added in May, 1969.
In Sept. 1971 discharge from hospital or leave was broadened to include day care or boarder.
At the end of each month the following were totaled:
Patients dismissed
Patients transferred out
Patients dismissed from leave
Total hospital days
Average length of stay
The above information was either written at the bottom of the page for that month or written on a separate sheet of paper & placed in the log book for that month. Also not all the above information was included each month.
Pages taped in back of log:
patients under age 21 discharged & average length of stay.
patients age 21 & over discharged & average length of stay.
patients discharged to Veterans Administration Hospital.
