Starting in August of 1939 a column for the ward the patient was admitted to was added.
Starting in May of 1940 a column was added for social security #.
Discharge information:
Date, no., name, diagnosis, condition, reason, age on discharge, years, months & days in hospital.
Death information:
Date, no., name, date of admission, age, nativity, diagnosis, age, time in hospital (years, months & days), cause of death ,buried here, autopsy.Collection
