Indicate to your district office personnel or local agent any desired changes and/or corrections in spelling, trade name or address. Any changes to be made will be done by district personnel.

You presently hold a Missouri liquor license as described above. This is your notice that application to renew this license must be filed with the Division of Liquor Control office in your district.(SEE RESERVE SIDE OF THIS NOTICE FOR THE ADDRSS OF YOUR DISTRICT) on or before May 1. Failure to file renewal by that date may result in delay in issuing your new license.
Under no circumstances will applications be hand carried to this Division by any applicant. . . . . . . . . None may be picked up personally at this office
