This record was given voluntarily & contains the following information:
-a 2 page handwritten narrative by the patient entitled "Pre St
Joe"
History" This narrative talks about his growing up years at home.
-a 12 page handwritten narrative by the patient entitled "Regarding Stigma". This narrative talks about his stay at St. Joseph Hospital # 2, the job he had leading up to his voluntary
hospitalization, and the jobs he had after being released from
the hospital.
-copies of the following: admission face sheet, medical
certificate, application for voluntary hospitalization, admission
note, physical exam, medication record, social service report,
therapy progress notes, doctors progress notes, nursing
admission & observation notes, a narrative summary done at
Western Missouri Mental Health Center where the patient had
been receiving care before admission to St. Joseph State
Hospital # 2, correspondence to & from family, vocational
rehabilitation correspondence concerning jobs.
Also included in this folder are copies of 1983 St. Louis newspaper articles concerning the killing of Alvin A Segelbohm,
the retired owner of Alvin Optical Co. The above patient had evidently worked for Mr. Segelbohm at Alvin Optical after leaving St. Joseph State Hospital # 2. Mr. Segelbohm's death occurred 10 years after this patient left Alvin Optical.Collection
