This is pale buff stationery with "Samuel Hayden Maddox, MD." in medium blue print centered on the top.
The cursive writing in pencil tells us that George Hayden was a member of Company B of the Maryland Battalion attached to James R. Herbert's regiment attached to Brigadier General George Stewart's brigade attached to Major General Edward Johnson's division attached to General Ewell's ??. George Hayden was wounded on July 3rd, the third day of the battle of Gettysburg during the skirmish for Culp's Hill.
