This book covers Spring term 1887 to Spring term 1891. One page of instructions is followed by pages ruled off into twelve five-day weeks with a statistical summary section on the right side. A pair of pages was sufficient for a term unless there were more than thirty-three students, but the classes were large and often used more than one pair of pages. A grade by grade list of students for an unknown quarter is written on the example sheet after the instructions. The entries for the Spring term 1891 are crowded onto the last pair of pages.
Regina Hammett identified this school as the Hollywood School.
