This printed form's first page is a summary of the number of schoolhouses, number of teachers and principals, length of school terms, numbers of pupils, attendance figures for various terms, and school houses built or enlarged. Page two asks for the number on the roll and average daily attendance for each term for each school, plus number of students in each grade and the subjects studied. Page three asks about expenses. Pages four and five provide more room for this information. Pages six and seven ask for the same information for colored schools. The final page is a statement of receipts and disbursements.
