1 folder ; 11 3/4" x 9 1/2" These forms were used to assess the condition of several stones in the Saint Nicholas Cemetery. All the assessments were done on January 9, 2004 and all were done by Andy Del Gallo. There is a color photo at the top of the first page for each stone. There were lines for location, name, condition, conservation priority, stone type, memorial type and size, with larger spaces for notes about inscriptions, physical condition, and conservation recommendations. On the second page, there was space for two conservation proposals, including cost.
