Form, Application for Leave of Absence, Cecil M. Burdick. Black printed text on white/cream paper. Form titled "APPLICATION FOR LEAVE OF ABSENCE" ("FORM S-9727B"). Two circular and one rectangular "RECEIVED" stamps are stamped in purple ink on the back of the form. The form was filled out on May 21, 1918 to request Cecil's leave of absence from her work as Chief Operator at The S.W.T.&T. Co. (Houston, Texas) to do work overseas with The Salvation Army during World War I. The period of time for which she requested her leave of absence was "June 1, 1918 to Nov 30, 1918". "A/O COL. BARKER, LIGNY-EN-BARRIOS, MEUSE, FRANCE" is written down as her address during absence. Full response filled out for "Reason for Requesting Leave" reads: "TO DO SALVATION ARMY WORK IN FRANCE".
