Scope and Content

Copy of Cecil May Burdick's birth certificate. Form number 6874: "TEXAS DEPARTMENT OF HEALTH/ BUREAU OF VITAL STATISTICS/ CERTIFICATE OF BIRTH". The copy was created in 1942 under W.D. Miller, Clerk County Court, Harris County, Texas. It includes Cecil's birth information, a signed "Affidavit A" section, and a signed "Affidavit B" section. The paper's front is black with printed and handwritten text in white. The back of the paper is white and contains the official stamp of approval, with lettering stamped in red and filled out/signed using a pencil.
