Scope and Content

Form, Extension for Leave of Absence, Cecil Burdick. Typed black text on white/cream paper. The actual title of the form is "APPLICATION FOR LEAVE OF ABSENCE" ("FORM S-9727B"), but on this particular piece of paper "APPLICATION" has been crossed out in black ink with "EXTENSION" written above. Two circular and one rectangular "recieved" stamps are printed on the back in purple ink. It appears that someone filled out this form on behalf of Cecil, and that it was a follow-up form/message after having already sent an initial application and not hearing back. The form was filled out on December 12, 1918 to extend Cecil's leave of absence from her work as Chief Operator at The S.W.T.&T. Co. (Houston, Texas) due to her work overseas with The Salvation Army during World War I. The period of time for which she requested to extend her absence was "Dec 1918 to May 31, 1919." Her Address During Absence is listed as "Ligny-en-Barrois, Meuse, France/ a/o Salvation Army." Full response filled out for "Reason for Requesting Leave" reads: Front page: "MISS BURDICK IS IN FRANCE DOING/ SALVATION ARMY WORK. I HAVE SENT HER EXTENTION/ FORMS TO BE SIGNED BUT [???] HAS NOT BEEN/ OVER/" Message continued on the back: "RETURNED. I KNOW THAT MISS BURDICK/ WISHES TO REMAIN IN THE SERVICE OF THE/ COMPANY AND WILL RETURN TO DUTY AS/ SOON AS HER SERVICES ARE NO LONGER/ NEEDED AT THE FRONT."
