state of Ohio County of Trumbull
Nelson Bailey aged 50 years a resident of warren
County of Trumbull state of Ohio who being duly
Sworn according to loan Declares that he is a
persioner of the united states by certificate number
872. 559 and duly enroled at the pension agency
at the rate of Six Dollars Per Month under the act
of June 27, 1890 = incured in the Military
Service of the United States while serving
as 1st sergeant in Company Oh. 29 rgt Ohio state pay army
That his present phisical condition is
Such that he Blieves himself entitled to incres
an increased Pension
He further declares - that his is entitled to incres
under the old law

Aplication is also hereby [?] for increes of pension on account
of a liability to [?]- at or near shelmount in state of
Tennesee [?]- [?]- 17 May to 1864 he contracted Diseas of
liver which has troubled him ever since and
resulted in Diseas of Stomach [?] He is now
satisfied that what physicians have recently
pronounced Disease of Liver in his case is
the same disability which he contracted as a

to be Inflammation of the Bowels Becaus it caused pain
in the abdomen he asks that this application
