Blue-and-white transfer-printed plate with the image of a house in the center and four buildings around the border. The central image is labeled, "HOME OF PRESIDENT COOLIDGE PLYMOUTH VERMONT". The vignettes of buildings around the border are separated by flowers. The back is labeled, "THE ADAMS SOUVENIR SERIES" along with further production details.
