Six copies Undated Tax Levy Report form of School District Clerk to assess taxes for the benefit of the schools. This report must be sent to the town, village or city clerk on or before September 1 in each year. It is subject to amendment on or before the 3rd Monday in November as provided by sec. 40.16(10). TAX LEVY REPORT OF SCHOOL DISTRICT CLERK TO THE TOWN, VILLAGE OR CITY CLERK To , Clerk of the town, village or city of , county of, state of Wisconsin : I , School District Clerk of , District No. , of the town, village or city of , county of , do hereby depose and say that the sum of Dollars was voted by the electors of the above named district, at the annual (or special) school district meeting, held on the day of , 19___ (and, in case the district is joint) the proportion of such sum that must be raised in that part of said joint district lying in your town, village or city, is Dollars, which you are hereby respectfully requested to assess against the taxable property of such district lying in your town, village or city, as required by Subsection (9) Section 40.11 of the statutes. Section 40.11 (9). The district clerk shall deliver to the town clerk, before September 1 in each year, a certified statement showing the amount of taxes voted and not before reported, and also the amount of tax to be collected in such year, if any, for the annual payment of any loan. In case of a joint dis- trict, he shall deliver to the clerk of each municipality in which any part of the district is situated, a certified statement showing the proportion of such taxes to be assessed in that part of the district within his municipality, which proportion shall be ascertained from the full valuations as last certified to him by the county superintendent from the certification made to the county super- intendent by the state superintendent from the certification of the full valua- tions of each part of a joint district made to the state superintendent by the state department of taxation under section 40.87 (2).
