This wooden plaque has a cast metal medallion beneath a metal inscription plate. The inscription plate reads: "In appreciation Em2 (SS) Ted Dubay, USN for a job well done on board the USS Henry Clay SSBN 625 (Gold) Mar.1969 to Mar. 1971. Aloha". The medallion depicts a submarine surrounded by two heraldic dolphins. There is an atom surrounding the earth at the bottom front of the submarine. At the top left is a constellation and at the top right is a star. Surrounding these images is a ring with the words: "preservio et natio USS Henry Clay SSBN 625".
