"Report of a Contagious Disease," Department of Health, City of Los Angeles, February 1914. The postal card has lines for reporting the name of a person, address, age, gender, date of illness, type of disease from a list given above, occupations of adults in the family, the name of the school at which persons at the address attended, and the informant's name. The card is addressed on the other side to the Health Commissioner at city hall and there is a preprinted one-cent stamp. The unused card has very minor edge rubbing and extremely light soiling and is in good condition.
