JOSEPH AVENUE" "EVANSVILLE 12, INDIANA" "No. 57367" "ON REORDER GIVE ABOVE NUMBER"; there is also a box that says "DON DAILEY & ASSOCIATES" "123 LOCUST ST." "EVANSVILLE 8, IND.". There is a large yellow glue stain on the back as well.
