Embossing on the front reads: 'PAUL REVERE/LIFE INSURANCE CO./WROCESTER, MASS./ HEALTH-ACCIDENT-LIFE" with a faint depiction of Paul Revere riding a horse in the center of the tag. The reverse side is embossed with: "1164610/THIS WILL IDENTIFY/THE OWNER/PLEASE NOTIFY THE/PAUL REVERE"
